Redstone @ Home is currently seeking an RN Triage Coordinator. Hiring for a Full Time position to work 7 days ON and 7 days OFF schedule. The position would work 4 pm - 9 am during the week and 7 am - 7 pm on Saturday/Sunday. The position is OFF every FRIDAY. Company-issued Subaru for to-and-from work commute, plus visits!

What will I do as an employee with Redstone?

As a Triage Coordinator (RN), supporting the Redstone mission will include the following responsibilities:

  • Conducts visits for nursing care to clients in a variety of settings. Responsible to conduct admission assessments and OASIS for Home Health clients that require after-hour initiation of services, due to staffing needs or timeliness of acute or post-acute discharge. Participates timely in the quality review of admission documentation.
  • Triages all phone calls for client needs in all departments: Hospice, Home Health and Home Care. Recognizes the opportunity to be the after-hours hospice expert and a key consultative resource in all care environments, affording patient's access to hospice care if clinically appropriate. The program will have RN Case Managers in reserve call Home Health and Hospice, if visit capacity requires after-hour support.
  • Responsible for the implementation of services to meet client needs and follows current nursing professional standards of practice and agency approved protocols for hospice. Provides ongoing assessment of clients, assuring an accurate evaluation of client needs. Identifies with clients need or quality for additional services and makes referral to appropriate IDT member. Documents the timely entry of patient evaluation/assessment into the electronic medical record.
  • Maintains and grows all professional relationships with all referral sources including but not limited to Redstone Highlands continuum of care, community physicians, physician practices, community environments of residential care and the acute care hospital representatives.

What do I need for this role with Redstone?

  • A license in the state of Pennsylvania as a Registered Nurse (RN).
  • Two years (2) of experience in Home Health, Home Care or Hospice (preferred).
