Redstone@Home is seeking an RN Case Manager - Hospice, to work Full Time - Days, servicing patients primarily in Westmoreland County.

Our Skilled Nursing Center, Redstone Highlands Healthcare Center, is nationally recognized as a High Performing facility in U.S. News and World Report's 2020-2021 list of Best Nursing Homes.

In this position you will be provided a company car and travel to patient homes to conduct case management.

Home office will be in Greensburg, however, you will not be expected to report to the office on a regular basis.

Team meetings will occur weekly (on Thursdays) at our Greensburg office.

Sign on Bonus: $10,000.

What will I do as an employee with Redstone?

As an RN Case Manager for Hospice, supporting the Redstone mission will include the following responsibilities:

• Planning, coordination and delivery of quality home health care, hospice care and wellness services, assuring compliance with corporate standards and federal, state and local regulations
• Provides nursing care to clients in a variety of settings
• Implementation of services to meet client needs
• Follows current nursing professional standards of practice and agency approved protocols for home health, hospice and home care
• Provides ongoing assessment of clients, assuring an accurate evaluation of client needs

What do I need for this role with Redstone?

• Current Registered Nurse license in Pennsylvania (BSN preferred)
• CPR certification
• Prior community based experience in a Medicare-certified home health and/or hospice agency preferred or palliative care preferred.
• Basic computer literacy with experience in Windows-based programs preferred.
